They're clearly outlined in the report I linked to in the OP.

Tusla, the child and family agency, will begin the recruitment process for a new CEO after incumbent Fred McBride made the shock announcement that he is leaving the post at the end of September.

The Charleton Report is damning in its criticism of Commissioner Callinan and Suprintendant Taylor.

If this pair managed to reach the upper echelons of the organisation, how many more of their ilk remain employed at various levels of seniority?

It outlines that Ms Fitzgerald and her department rightly did not attempt to direct the commissioner as to how she should approach the O'Higgins Commission"
Fitzgerald and a large group of her DoJ officials saw the explosive email the handling of which led to Fitzgerald's spot of bother. She apparently forgot about it as soon as she saw it, and not a single one of them saw fit to make a note of it. Not a word about it.

Do you actually believe that to have been the case?

Both of them have given explanations for this decision: that David Taylor was talented, experienced, articulate and so on. He is not. All of this is just plain untrue.”

In one section, the judge accuses Taylor of “daft” evidence.

He said: "The Tribunal is convinced that Superintendent David Taylor did more than merely inform some journalists that in the past there had been an investigation into Maurice McCabe and that the Director of Public Prosecutions had decided that there should be no prosecution and that he was consequently embittered. That is all he claims to have done. That is untrue."

The Tusla role seems to be still unexplained. It's unconscionable and positively Orwellian that a state organisation could concoct an allegation like this about someone. It's essential that judge Charlton gets to the bottom of that.
