No body is held accountable for their errors, omissions and corruption. Or so it seems.
Quite right and this is one of the core problems. If you accept responsibility, recognise your mistake, account for your actions then you can attempt to rectify/change the out-come.

I think it has to do with appointments, once you recieve an appointment, (any of your mentioned positions) then it seems you are accountable to no one and negligence/lack of experience/inability to read & understand everyday reports/lack of sound judgement/dis-regard to public concern/
we hear silly reasons like there was an "error in communication"
or excuses like 'he was the consultant in charge' (when mutilating women)
'i was not informed/aware' ! (but that is your job)

Politicians are never held accountable, whether we have Mary Harney making well judged slips about CJ that obviously would and did prejudice fair trial, to JOD and Trevor who takes clientelism to a new level by interfering in the judicial process. The HSE is not accountable and those that it funds are also not accountable.

Being accountable goes against the grain of being Irish I guess.
Private sector is accountable because they are employed rather than being appointed to position, however if the private sector worker makes a 'mistake' they loose their position. Public sector is huge & great distance between the lower level worker, managers and senior civil servants means the lower level worker is the first to be held to account for his/her labour, it remains to be seen if this accountability will go any further up the public sector ladder, even the lowere level council workers held no real account untill relatively recently.

Well, re-electing the same politicans and being surprised when they yet again act the same way has to be a major example. After all, that means the politicans are not suffering any deteriment. Problem with this though is the real scandal is that we - the electorate - are stupid enough to do it again and again.
