    Monitoring and assisting the most vulnerable patients in the community should vastly improve the efficiency of health care delivery and cut hospital admissions from present levels. That would help end the never ending Irish hospital trolley counts. As a semi nationalised system, the HSE should be able to implement a similar programme to the NHS's Vanguards. This challenge should provide a test of the HSE's capacity for change management.
